Property and notice summary
The report should begin with the property address, client contact, known unit count, walkthrough date and any LAHD inspection date supplied by the owner. This prevents findings from becoming detached from the actual deadline and scope.
- Property address and basic building information supplied by the client.
- The document received: SCEP Notice of Inspection, Order to Comply or preventive request.
- Accessible areas included in—and excluded from—the private walkthrough.
- Known deadline and immediate access constraints.

Photo-documented visible findings
Each observation is more useful when it identifies the location, visible condition and supporting photo. The language should describe what was observed without pretending to make LAHD’s compliance decision or an engineering conclusion.
- Exact location: unit, room, common area or exterior elevation.
- Concise description of the visible, accessible condition.
- Photo reference and date of observation.
- A note when access, concealment or specialist evaluation limits the review.

Priority and responsible path
A long defect list is not yet a plan. The report should distinguish urgency and identify the likely next owner of the item—maintenance, a licensed trade, records research, permit evaluation or a design professional.
- Immediate attention: active hazard, active leak or escalating damage.
- Before inspection: visible correction that can be scheduled within the available window.
- Further evaluation: scope cannot be responsibly defined from a visual walkthrough alone.
- Apparent permit or design flag: records, plans or agency direction may control the next step.

Recommended next action—not a pass certificate
For each finding, the report should state the next practical action: repair, test, records search, specialist review, permit check or monitor. A private report is not an official SCEP clearance, certification or guarantee of the future City result.
When construction is appropriate, a repair proposal should be separate from the assessment report. That makes the owner’s decision and the proposed scope easier to evaluate.

Completion evidence and remaining issues
If authorized work is completed, the readiness file can be updated with after-photos, invoices and applicable permit records. Items still awaiting access, approval or specialist input should remain visible rather than being marked complete prematurely.
- Before-and-after photo references for completed field work.
- Applicable invoice, trade or permit record supplied to the file.
- Open-item list with the responsible person and next date.
- A clear disclaimer that LAHD makes the final compliance determination.
